The clinic takes four kinds of claims for people in and around Clarksville. If yours is something else, we'll say so on the first call and name a place to try.

Bring what you have, not what you wish you had
A missing paper is something we can request. Start with whatever is within reach.
Every letter
The notice, the envelope it came in, and anything earlier from Social Security or the VA.
Your doctors
Names and rough dates for every provider, including clinics you stopped seeing.
Your work history
Jobs you've held in the last fifteen years, even short ones, and what each asked of you.
Your service papers
If you served, your DD-214 or a note of where your service dates are recorded.
